Please note on date: many sites give this as 6 November 1964 but this date seems to one erroneously attributed to this in the 1970s and transferred without question over the decades. It has been repeatedly questioned in recent years and given the printer's date of December and the chart date of January 1965 is unlikely. Please see the submission history for more.
